Q: What are the eligibility requirements?
If you are homebound and have a history of a stroke, Parkinson's, CPOD, Heart Failure, Alzeihmer's, ALS, Atrial Fibrilation, or other chronic illnesses you are eligible.
Q: What is Homebound?
A patient is considered "homebound" when the patient has difficulty in transporting to a doctor's office on their own. This difficulty can be due to physical and/or cognitive decline. Homebound is when it takes a taxing effort for the patient to leave their home in any capacity.
Q: Who performs my treatment?
A nurse practitioner will see the patient a minimum of once a month. If a skilled need or sickness arises there may be more frequents visits for the patient.
Q: Will Primary & Palliative Home Care accept my insurance?
Most insurances plans will cover primary and palliative treatment that you receive. Your insurance will be billed just like a typical visit to the doctor's office.
Q: Do you offer 24 hour care or overnight services?
Yes. Our services are available to our patients 24 hours a day and 7 day a week.
Q: What is the difference between palliative care and hospice?
Palliative care is mainly focused on relief from suffering. Whether you are a patient who is living with a chronic disease or is being treated for one. The patient does not have to be terminally ill to receive palliative care. The patient can continue curative treatment while receiving palliative care. We will not only address physical needs, but also mental, social, and spiritual needs.
Hospice care on the other hand is available to terminally ill patients. It prepares the patient and the family for the patient's end of life. When treatment for the illness is no longer pursued, hospice care also relies on a family caregiver and a hospice nurse.
